Any changes to your reservation must be made via email to outreach@obt.org no later than one week prior to the performance. Your change or cancellation is not confirmed until you receive a response.

NOTE: In an attempt to cut down on paper usage and mailing costs, we are transitioning to providing our teacher packets online, which you can then download and print, as needed. We will e-mail you a link which can be used to retrieve the packets from our website. If you would like us to send you a hard copy packet in the mail, please indicate the number of packets you will need.

Upon receipt of this form, we will send a confirmation. Invoices will be sent via mail and must be paid one week prior to performance, or you may lose your reservation.

Please note - Due to high demand for seats, we often have a waiting list. It is imperative that you contact OBT if you need to cancel your reservation so another group may attend.
